The Smart Care 360 App facilitates active engagement in our preventive health and wellness initiatives. Users can conveniently track their activity levels, quality of sleep, weight, and other metrics related to their wearable devices through the application. Additionally, it allows for the seamless integration of data from Apple's Health app, including step count information from iPhones. Users have the option to manually input data directly into the Smart Care 360 App.

Beyond tracking personal health metrics, the app offers features such as communication with health coaches and care teams, setting reminders, engaging in wellness challenges, establishing private connections with peers, and more.

It is important to note that the Smart Care 360 App does not provide diagnostic or treatment services. For assistance with registration or app setup, individuals are encouraged to reach out to their program coordinators.
